Instructions for Intake Forms

• Complete Both Pages of New Client Form. If coming for Couples or Family Counseling, please fill out a form for each individual.
• Read and Sign the "Counselor/Client Agreement". Please discuss any questions or concerns you have with your counselor.
• Know Your HIPPA Privacy Rights. This form is for you to keep and does not need to be returned to your counselor.
• Bring Forms to First Appointment (or email).Please bring the completed paperwork with you to your first appointment.
